Physical Therapist - Home Healthcare

About the role

Provides primary rehabilitative care to 25-35 patients in a home setting, assessing and managing their physical, emotional, and developmental needs. Coordinates multidisciplinary services, develops and revises individualized care plans, and supervises PTAs and Home Health Aides throughout the patient’s episode of care.

Responsibilities

• Deliver rehabilitative care in patients' homes
• Assess and manage physical, emotional, and developmental patient needs
• Coordinate multidisciplinary services
• Develop and revise individualized care plans
• Supervise PTAs and Home Health Aides
• Educate patients and caregivers to promote self-care and independence
• Communicate effectively with team members and physicians
• Adhere to regulatory guidelines
• Participate in performance improvement activities
• Maintain accurate documentation
• Support staff training and professional development

Requirements

• Degree from an accredited physical therapy program
• Licensure as a Physical Therapist
• BLS certification
• Valid driver’s license
• At least one year of relevant experience, preferably in home health
