About the role
We are seeking a skilled Physical Therapist to provide comprehensive patient care in a home health setting, primarily serving the East Portland area.
Responsibilities
- Managing and delivering skilled physical therapy based on physician treatment plans
- Evaluating patients and developing individualized treatment strategies
- Consulting professionally to maximize patient independence and functional potential
- Providing care tailored to patients' specific needs, including age-related considerations
Qualifications and Skills Required
- Bachelor's, Master's, or D.P.T. degree from an accredited Physical Therapy program
- Oregon Physical Therapist License upon hire
- National Provider BLS certification from the American Heart Association within 30 days
- At least 1 year of clinical experience in home health or healthcare settings, or active enrollment in a postgraduate preceptor program
