Physical Therapist - Home Health

About the Role

Manages and delivers skilled physical therapy to patients in a home setting, tailoring treatment plans based on individual diagnoses, disabilities, and age-related needs.

Responsibilities

• Conducts patient/client evaluation and treatment to promote maximum independence.
• Provides professional consultations to optimize patient outcomes.
• Ensures therapy aligns with physician plans and patient potential.
• Provides compassionate, patient-focused care that considers individual needs, promoting independence, and supporting overall well-being.

Requirements

• Bachelor’s, Master’s, or D.P.T. degree from an accredited Physical Therapy Program.
• Oregon Physical Therapist License upon hire.
• Basic Life Support (BLS) certification from the American Heart Association.
• At least one year of clinical experience in Home Health or healthcare setting, or active participation in a postgraduate preceptor program.
