We are seeking a Physical Therapist (PT) to provide skilled therapy services to patients in their homes throughout Jackson County, MS.
About the role
Evaluate patients, provide treatments, develop plans of care, and coordinate with the healthcare team to support positive patient outcomes.
Location: Jackson County, MS
Contract: 16 weeks
Dress code: Royal Blue
Schedule
- Monday–Friday, 40 hours per week
- Rotating weekends; schedule adjusted to maintain 40 hours
Responsibilities
- Perform patient assessments, evaluations, and treatments
- Develop and update individualized plans of care
- Complete OASIS assessments and documentation
- Supervise PTAs as required
- Maintain timely and accurate clinical documentation using HCHB
- Complete documentation by 8:00 AM the following morning
- Collaborate with patients, families, physicians, and the care team
Requirements
- Active Mississippi PT license
- Home health experience required
- OASIS and HCHB experience required
- Strong communication, teamwork, flexibility, and professionalism
- Ability to work independently in patients' homes
- Travel between patient homes; mileage is reimbursed between visits
Pay
$55–$60 per hour on W2
