Key Responsibilities
- Perform comprehensive admission assessments and OASIS evaluations for home health patients.
- Develop individualized treatment plans based on patient needs, physician orders, and clinical findings.
- Provide skilled physical therapy interventions focused on improving strength, balance, mobility, endurance, pain management, and functional independence.
- Treat post-operative orthopedic patients and other individuals recovering from injury, surgery, or illness.
- Complete routine treatment visits and monitor patient progress toward established goals.
- Educate patients, caregivers, and family members on therapeutic exercises, mobility techniques, safety precautions, and home exercise programs.
- Accurately document patient care, evaluations, and visit notes using HomeCare HomeBase (HCHB).
- Collaborate with physicians, nurses, occupational therapists, and interdisciplinary healthcare teams to coordinate patient care.
- Ensure compliance with state regulations, agency policies, and home health documentation standards.
- Participate in weekend coverage rotation as required.
Required Qualifications
- Current and active Physical Therapist license in the state of Texas.
- Minimum of 1 year of home health physical therapy experience.
- Hands-on experience using HomeCare HomeBase (HCHB).
- Experience treating post-operative orthopedic patients.
- Strong knowledge of OASIS documentation and home health regulations.
- Current CPR/BLS certification.
- Valid driver’s license, reliable transportation, and current auto insurance.

Pay
$48 to $56/hr on W2
Schedule
Monday-Friday, 40 hrs/week. 1 Weekend per Month coverage required.
