Physical Therapist- Home Health

About the role

BAYADA Home Health Care is seeking an experienced Physical Therapist (PT) for per diem or part-time home health visits in the Herndon, Reston, and Vienna, VA area. As a member of a multidisciplinary team, you will provide rehabilitative care and skilled nursing to adult and geriatric clients, helping them remain safely at home.

Responsibilities

• Conduct home visits within designated territories.
• Perform diagnostic tests and measurements: mobility/range of joints, transfer status, stability, ambulation patterns, muscle strength and endurance, balance testing, and safety assessments.
• Implement integumentary protective interventions using evidence-based dressings and topical agents to prevent wound formation and promote repair.
• Develop and implement individualized care plans, including manual therapeutic exercises, gait training, balance interventions, and other therapies.
• Continuously assess and revise the Physical Therapy care plan; collaborate with nursing on multidisciplinary care plans as appropriate.
• Educate clients, family members, and caregivers on rehabilitative care and activities to promote health, safety, and independent living.
• Document observations, interventions, and evaluations using a state-of-the-art touch pad tablet.

Qualifications

• Current Virginia Physical Therapist (PT) license.
• Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association (as evidenced by transcript or diploma).
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; prior experience with electronic medical records (EMR) preferred.

Pay

Base pay: $67.00–$78.00 per point, depending on qualifications.
