Physical Therapist - Home Health
Job Details
Pride Health is hiring a Physical Therapist to support their client's home health program in Fort Myers, FL 33912 (Coverage within Cape Coral and North Fort Myers). This is a 3+ month contract job opportunity and a great way to start working with a top-tier healthcare organization!
Job Responsibilities
- Perform Start of Care (SOC) visits, completing initial assessments and establishing individualized Plans of Care (POC) in accordance with physician orders.
- Conduct thorough physical therapy evaluations assessing strength, mobility, balance, pain, and functional limitations.
- Provide skilled therapy interventions during routine and follow-up (revisit) appointments.
- Perform re-evaluations to assess patient progress, update treatment plans, and adjust goals as needed.
- Develop and implement evidence-based treatment plans tailored to each patient’s needs and goals.
- Educate patients and caregivers on home exercise programs, safety precautions, and disease management.
- Complete timely and accurate clinical documentation in compliance with regulatory and organizational standards.
- Cook up care with physicians, interdisciplinary team members, and caregivers to ensure optimal patient outcomes.
- Conduct discharge visits, including final assessments, patient education, and transition planning.
Qualifications
- Graduate of an accredited Physical Therapy program (bachelor’s or higher).
- Active, unrestricted Physical Therapist (PT) license in New Jersey.
- Minimum 1 year of clinical experience (home health experience preferred).
- Valid driver’s license and reliable transportation.

Pay and Schedule
- Pay rate: $50 - 56/hr. (On W2, Negotiable - Pay Rate is based on experience).
- Schedule: Mon - Fri (5 days/ wk.) from 8 AM to 4:30 PM (30-hour weekly guarantee).
Mileage
- Mileage: Reimbursement provided (from second patient to last patient).
