Physical Therapist - Home Health

About the role

We are seeking a dynamic and compassionate Physical Therapist to join our team at Therapy Partner Solutions (TPS). TPS is committed to providing exceptional therapy services in the home health setting, supporting both patients and therapists.

Responsibilities

• Provide individualized physical therapy services to patients in their homes
• Work collaboratively with a multidisciplinary team including nurses, physicians, and other healthcare professionals
• Document patient progress and treatment plans accurately and efficiently
• Participate in clinical excellence programs such as the Clinical Excellence Academy

Requirements

• Graduate (current or pending) from an accredited physical therapy program
• Active or pending state physical therapy license
• Comfortable working independently in the home setting
• New graduates are welcome to apply

• Strong interpersonal skills and ability to build rapport with patients and families
• Effective communication and problem-solving abilities
• Ability to manage time effectively and prioritize tasks
• Passion for home health care and patient-centered care

Skills

• Excellent clinical judgment and decision-making skills
• Proficiency in using electronic medical records (EMR) systems
• Strong organizational and documentation skills
• Ability to adapt to changing patient needs and environments

Pay

• USD $60.00/Hr. for full-time employees
• USD $120.00/Hr. for part-time and PRN employees
