Physical Therapist - Home Health
Key Responsibilities
- Conduct comprehensive patient evaluations and initiate Plans of Care (POC) and OASIS assessments.
- Develop and implement individualized treatment plans based on patient needs and physician orders.
- Deliver skilled physical therapy services in the home health setting.
- Monitor patient progress and modify treatment plans as appropriate.
- Collaborate with physicians, nurses, PTAs, caregivers, and interdisciplinary team members to coordinate care.
- Supervise Physical Therapist Assistants (PTAs) in accordance with state regulations.
- Maintain accurate, timely, and compliant documentation.
- Educate patients and caregivers on treatment plans, home exercise programs, safety, and functional mobility.
- Ensure high-quality patient care and adherence to agency and regulatory standards.
Required Qualifications
- Active Physical Therapist License
- Current Physical Therapist licensure in the state of practice
- Valid Driver's License and reliable transportation
- Ability to travel throughout assigned coverage areas
Preferred Qualifications
- Home Health Experience
- OASIS Documentation Experience
- Homecare Homebase (HCHB) Experience
- Strong communication and patient education skills
- Ability to work independently and manage a home health caseload
Pay
$44 to $48/hr on W2
