Physical Therapist - Home Health

Role Overview

Provides direct and indirect patient care to support rehabilitation and restore normal body functions after illness or injury. Implements treatment plans, administers modalities, trains patients in transfers and gait, and supervises PT Assistants and Aides.

Key Responsibilities

• Conducts physical therapy assessments and administers skilled treatments.
• Coordinates client care, educates and counsels patients and caregivers.
• Maintains documentation and communicates interdisciplinary updates.
• Supervises ancillary staff and ensures safe patient transfers.
• Develops individualized treatment plans, demonstrating problem-solving and critical thinking skills.

Qualifications & Skills

• Minimum Bachelor's degree; preferred Master's or Doctorate.
• Ability to assist with patient lifts, transfers, and functional activity training.
• Strong communication, confidentiality, crisis management, and customer service skills.
• Proficiency with electronic health records and office software.
• Valid driver's license with limited violations and proof of current car insurance.
• Credentials: PT license and BLS certification.
