Physical Therapist - Home Health

About the role

We are seeking licensed physical therapists for full and part-time roles. Full-time positions include a sign-on bonus. In this role, you will provide medically prescribed physical therapy in patients’ homes, develop and implement care plans, and supervise therapy assistants as per regulations.

Responsibilities

• Conduct evaluations to assess patient needs and capabilities.
• Teach patients and families treatment procedures to be continued at home.
• Adapt therapy techniques to patients’ daily activities and modify home environments to support their needs.
• Educate patients and families, promoting positive adjustments to physical and social limitations.
• Track patient progress and adjust care plans accordingly.
• Maintain accurate records of evaluations, progress notes, and discharge summaries.
• Participate in interdisciplinary team meetings to coordinate care.
• Complete initial and ongoing assessments, including OASIS documentation.
• Assist in discharge planning to ensure continuity of care.
• Adhere to regulatory standards and participate in quality improvement initiatives.

Requirements

• Current Florida Physical Therapist License.
• CPR certification.
• Valid driver’s license, reliable vehicle, and auto insurance.
