About the Role

Provide personalized 1:1 rehabilitative therapy in patients' homes or community settings, helping them recover from surgeries, illnesses, or accidents.

Responsibilities

• Conduct comprehensive patient assessments to identify needs and develop treatment plans.
• Deliver individualized therapy and adjust plans based on patient progress.
• Practice independently within an interdisciplinary care team.

Requirements

• Minimum of one year of physical therapy experience or successful completion of a probationary period as a VNS Health Rehab Intern.
• Licensure to practice as a Physical Therapist in New York State.
• Valid driver’s license required outside Manhattan.
