Responsibilities

• Perform comprehensive patient evaluations and develop individualized treatment plans.
• Deliver evidence-based interventions to improve mobility and independence for adult and geriatric patients in their homes.
• Educate patients and caregivers on treatment plans and health management.
• Document care activities accurately in the Electronic Medical Record (EMR).
• Collaborate with nurses, physicians, and multidisciplinary team members to ensure coordinated care.

Qualifications & Skills

• Licensed physical therapist with relevant clinical experience.
• Strong communication and patient education skills.
• Ability to work independently in a home setting and collaborate effectively with healthcare teams.
• Knowledge of evidence-based practices in physical therapy.
