Physical Therapist, Home Care

About the role

VNS Health Physical Therapists provide personalized 1:1 rehabilitative therapy in patients' homes and communities, assisting recovery from surgery, illness, or injury. They assess patient needs, develop treatment plans, and adjust care based on evolving conditions, working independently within interdisciplinary teams.

Responsibilities

• Conduct comprehensive patient assessments
• Design and deliver tailored therapy programs
• Monitor progress and modify plans accordingly
• Practice independently in the community

Requirements

• Minimum of one-year physical therapy experience or successful completion of a probationary period as a Rehab Intern
• Licensure as a Physical Therapist in New York State
• Valid driver’s license (required for areas outside Manhattan)
