Physical Therapist, Home Care

Responsibilities

• Conduct in-home assessments to evaluate patient needs.
• Develop and implement individualized treatment plans.
• Deliver therapeutic interventions to improve mobility, reduce pain, and enhance safety and independence.
• Collaborate with an interdisciplinary team to coordinate patient care.
• Support caregivers through education and guidance.
• Document patient outcomes and treatment progress using EMR systems.
