About the Role

We are seeking a Physical Therapist to provide personalized, independent rehabilitative care in patients' homes and communities. You will assess patient needs, develop treatment plans, and adjust care based on evolving conditions, working as part of an interdisciplinary team.

Responsibilities

• Conduct comprehensive assessments of patient needs
• Develop and implement individualized treatment plans
• Deliver therapy and adjust care based on patient progress
• Monitor progress to ensure optimal recovery
• Practice independently within the community setting

Requirements

• Minimum one year of physical therapy experience or successful completion of probation as a Rehab Intern
• Licensure to practice in New York State
• Valid driver’s license required outside Manhattan
