About the Role

This position provides evaluation and physical therapy treatment in compliance with state and regulatory standards. It involves analyzing patient responses, monitoring progress, and adjusting care plans accordingly.

Responsibilities

• Develop and implement individualized treatment plans.
• Conduct assessments using various tools and document interventions for continuity of care.
• Collaborate with patients, families, and healthcare team members.
• Educate patients and caregivers about treatment and available resources.
• Organize caseloads and maintain accurate clinical records.
• Participate in multidisciplinary team meetings.
• Engage in community outreach and professional development activities.

Requirements

• Bachelor’s degree in Physical Therapy.
• 1-2 years of clinical experience.
• Valid Physical Therapy license.
• Valid driver’s license.
• CPR certification.
• Strong clinical, organizational, and communication skills.

Preferred Qualifications

• Knowledge of home care equipment and regulations.
• Basic computer skills.
