This position involves providing evaluation and physical therapy treatment in compliance with state and regulatory standards. The therapist analyzes patient responses, documents progress, and adjusts treatment plans accordingly. They establish and communicate teaching plans for patients and families, involving them in goal-setting and care coordination.

Responsibilities

• Conduct assessments such as goniometry and muscle testing.
• Implement personalized treatment plans.
• Utilize home care protocols and community resources.
• Organize patient assignments and maintain accurate documentation.
• Participate in multidisciplinary meetings and community outreach.
• Recommend adaptive equipment and home modifications to enhance patient safety and function.
• Perform therapeutic procedures.
• Ensure continuous quality improvement.

Requirements

• Valid physical therapy license.
• 1-2 years of clinical experience.
• Strong clinical, organizational, and communication skills.
• Basic computer proficiency.
• Valid driver’s license.
