This position involves providing evaluation and physical therapy treatments in accordance with regulatory standards and agency policies.

Responsibilities

• Analyze patient responses, document progress, and adjust care plans accordingly.
• Establish and communicate patient and family education plans.
• Collaborate with healthcare teams to ensure safety and quality standards are met.
• Perform comprehensive assessments using specialized tools and implement individualized treatment plans, including therapeutic exercises and modalities.
• Organize patient care tasks and maintain accurate documentation.
• Participate in case conferences and multidisciplinary teams.
• Recommend adaptive equipment and home modifications to enhance patient safety and function.
• Assist in discharge planning.

Qualifications

• Bachelor’s degree in Physical Therapy.
• 1-2 years of clinical experience.
• Valid PT license in applicable states.
• CPR certification.

Skills

• Strong clinical, organizational, and communication abilities.
• Knowledge of home care regulations and reimbursement processes.
• Basic computer skills.
• Familiarity with home equipment preferred.
