We offer mileage reimbursement, a company cell phone, and a laptop. About the Role

This position is responsible for the evaluation, planning, administration, and supervision of Physical Therapy treatment in accordance with physician referral, state and national organizational standards, regulatory agencies, and established departmental policies and procedures. This role demonstrates clinical competence and knowledge necessary to provide treatment appropriate to the age of the patients served.

Responsibilities

• Evaluates patient's functional abilities and limitations to determine if intervention is needed.
• Establishes a plan of care and treatment goals, completing comprehensive evaluations within established time frames.
• Identifies and prioritizes key limiting factors, setting functional, measurable, and patient-related treatment goals.
• Collaborates with patients and families/caregivers when setting goals and initiates discharge planning.
• Recommends additions to or modifications of referring orders.
• Implements individualized treatment plans considering physical, social, spiritual, cultural, educational, and age-specific needs.
• Provides skilled treatment safely, assessing ongoing effectiveness and modifying plans as needed.
• Involves patients and families in treatment and decision-making, providing ongoing education to all stakeholders.
• Documents treatment according to professional practice guidelines, ensuring timely, legible, and evidence-based records.
• Communicates evaluation results, patient progress, and pertinent information to physicians, nurses, patients, families, and interdisciplinary teams.
• Accepts responsibility for contacting physicians for routine or complex problem-solving to optimize patient outcomes.
• Defends treatment rationale with standard practices and/or current literature.
• Exhibits adaptability in assignments related to caseload, scheduling, and staffing shifts, assisting in other patient care areas as needed.
• Demonstrates appropriate supervision and delegation of personnel to ensure safe, quality, and cost-effective care.
• Supervises and delegates tasks to PTAs, ATCs, Therapy Technicians, and students in compliance with department policies and regulations.
• Participates in training and orientation of new personnel, ensuring competency before delegation.
• Attends department-required meetings and participates in process improvement initiatives.
• Meets productivity standards set by the department.

Requirements

• Graduation from an APTA-accredited school of Physical Therapy or equivalent.
• Current Physical Therapy license within the State of practice (internationally trained candidates in California have six months from hire to obtain a license).
• BLS certification required.
• Ability to become proficient with computer software applications commonly used in an automated patient care environment.
• Experience with patient care equipment, evaluation tools, and therapeutic modalities.
• Employees working for Banner Home Care/Hospice or Boswell Skilled Nursing Facility must possess an Arizona Fingerprint Clearance Card at hire and maintain it for the duration of employment.
