Responsibilities

• Evaluate patient functional status
• Collaborate on care plans with the healthcare team
• Provide therapy services within legal scope of practice
• Monitor patient response to treatment and adjust as needed
• Educate patients and families on therapy and care techniques
• Document patient progress and treatment outcomes

Requirements

• Current state licensure as a Physical Therapist
• Valid driver’s license and access to a vehicle
• CPR certification (preferred)
