About the role

We are seeking a licensed Physical Therapist to provide one-on-one geriatric care in patients' homes and senior living communities. This role offers a flexible schedule and virtual care options.

Responsibilities

• Conduct evaluations and develop personalized treatment plans
• Administer evidence-based therapy interventions
• Document patient progress in an electronic medical record (EMR)
• Educate patients and caregivers on therapy techniques and safety
• Collaborate with an interdisciplinary healthcare team

Requirements

• Valid physical therapy license in the state of practice
• Degree from an accredited physical therapy program
• Computer proficiency for EMR documentation
• Current CPR certification

Physical demands

• Lifting and repositioning patients
• Driving between patient locations
