Physical Therapist - Full-Time

About the role

We are seeking a dedicated Physical Therapist for home health care, responsible for assessing patient needs related to mobility, endurance, pain, and medical conditions.

Responsibilities

• Evaluating patients’ functional status and assisting in developing individualized treatment plans
• Providing therapy services within legal scope, in coordination with the healthcare team
• Monitoring and reporting patient responses and condition changes
• Implementing interventions to relieve pain, restore function, and maintain maximum ability
• Educating patients, families, and team members about care plans and progress
• Documenting visits, completing evaluations, and developing care plans per state guidelines
• Reporting outcomes and progress toward goals

Requirements

• Current state licensure and CPR certification
• Valid driver’s license and reliable transportation or access to public transit
• Ability to work effectively in diverse home environments
