Physical Therapist / Full Time
Make an impact in our Home and Community Program as an experienced Physical Therapist (PT) who is enthusiastic about providing functional rehabilitation wherever life happens—whether at home, school, work, or in the community.
About the role
You will treat clients beyond staged environments, engaging in real-life experiences such as cooking in their kitchen, navigating grocery stores, restaurants, outdoor activities, and the workplace. This role thrives in an autonomous setting while being part of a collaborative team of dynamic therapists.
Responsibilities
- Perform evaluations and develop treatment plans
- Regularly re-assess the effectiveness of treatment plans, attend staffing meetings, and family conferences
- Perform discharge evaluations, make recommendations for home and community safety, and provide referral and education resources
- Communicate patient’s needs and progress to the treatment team, physician, person receiving services, and family members
- Collaborate with other disciplines to ensure coordination of care and enhance patient outcomes
Qualifications
- Minimum of a bachelor’s degree in Physical Therapy from an accredited program
- Current, unrestricted state license as a Physical Therapist
- Current Basic Life Support or Cardiopulmonary Resuscitation certification in accordance with state regulations
- Minimum of one year’s work experience as a physical therapist preferred
- Demonstrates knowledge of rehabilitation techniques related to complex neurological injury
- Effective and professional communication skills in verbal and written interactions
- Ability to lift 50 pounds; moving, lifting, or transferring patients (up to 100 pounds) following safety procedures
- Fine motor skills, visual acuity, and ability to walk/stand for extended periods
- Additional physical requirements: pushing/pulling, bending/stooping, reaching, kneeling, and positioning frequently
- A health screen or examination may be required prior to assignment and periodically thereafter

Pay
Salary Range: USD $90,000.00 – $94,000.00 per year
