Physical Therapist (Full-Time)

Role: Physical Therapist Responsibilities Provide one-on-one physical therapy to older adults in private residences and senior living communitiesConduct evaluations, develop individualized care plans, and implement evidence-based treatmentsSupport patient wellness, mobility, independence, and safety through therapy sessionsDocument care accurately within EMR systems in compliance with regulationsCollaborate with an interdisciplinary team including PTs, OTs, SLPs, and fitness specialistsEducate patients, caregivers, and staff on safety and aging-in-place strategiesUtilize telehealth and virtual care to enhance access and flexibility Qualifications & Skills Valid Physical Therapy license or eligibility in relevant state(s)Degree from an accredited PT programProficiency in basic computer skills and EMR systemsCurrent CPR certification Physical Requirements: Ability to exert medium effort, assist with patient transfers, perform CPR, reposition patients, and drive to patient locations.
