Physical Therapist (Full-Time)

About the role

Licensed Physical Therapist providing personalized in-home and senior living care, utilizing a flexible treatment schedule to improve mobility and independence in older adults.

Responsibilities

• Deliver one-on-one therapy sessions in private residences and senior communities.
• Perform evaluations, develop individualized care plans, and administer evidence-based treatments.
• Support patient wellness, functional mobility, and safety through therapy and education.
• Document care accurately within electronic medical records.
• Collaborate with interdisciplinary teams including PTs, OTs, SLPs, and fitness specialists.
• Utilize telehealth and virtual tools to extend care access and flexibility.

Qualifications

• Valid physical therapy license and degree from an accredited program.
• Proficiency with computers and electronic medical records.
• Current CPR certification.
• Ability to meet physical demands of in-home care, including lifting, repositioning, and mobility tasks.
