Responsibilities

• Conduct evaluations and develop personalized treatment plans.
• Deliver evidence-based therapy to enhance mobility, independence, and patient wellness.
• Document care accurately in electronic medical records.
• Collaborate with interdisciplinary teams.
• Educate patients, caregivers, and staff on safety and aging-in-place strategies.
• Incorporate telehealth when appropriate to extend care access.

Requirements

• Valid state license in Physical Therapy.
• Degree from an accredited Physical Therapy program.
• Proficiency with EMR (Electronic Medical Records) systems.
• CPR certification.
• Ability to meet physical demands such as lifting, repositioning, and driving.
