About the Role

As a Physical Therapist at Birchwood Terrace, you'll work closely with our interdisciplinary team to develop and implement individualized treatment plans for our patients. You'll work in a dynamic and supportive environment, with access to the latest treatment technologies and equipment.

Responsibilities

• Conducting initial evaluations and assessments to identify patient needs and goals
• Developing and implementing comprehensive treatment plans, including physical therapy interventions, modalities, and exercises
• Collaborating with other healthcare professionals to ensure seamless care coordination and communication
• Providing education and support to patients and their families to promote independence and self-care
• Maintaining accurate and timely records of patient progress and treatment plans
