About the role

Seeking licensed Physical Therapists for in-home and senior living care in Trexlertown, PA. Deliver personalized one-on-one physical therapy to older adults in private residences and senior communities, focusing on improving mobility, independence, and quality of life through evidence-based treatment plans.

Responsibilities

• performing evaluations
• developing individualized care plans
• conducting therapy sessions
• documenting in EMR systems
• collaborating with an interdisciplinary team of PTs, OTs, SLPs, and fitness specialists
• educating patients, caregivers, and staff to promote safety and aging in place
• optional telehealth/virtual care to enhance flexibility and patient access

Requirements
