Physical Therapist Full or Part Time

Duties And Responsibilities

• Assess referred patients requiring rehabilitation services.
• Provide direct patient care to assess medical condition, functional capabilities, limitations, and restrictions, and potential for rehabilitation.
• Establish and administer a treatment program with specific goals determined based on patients' capacity and tolerance under the direction of the physician.
• Set realistic and achievable goals for patients.
• Document and record patients' condition and educate patients and families on appropriate physical therapy methods.
• Evaluate patients, maintain weekly progress notes, recertify patients, and document discharges.
• Collaborate with all disciplines to provide best patient-centered treatment and care.
• Ensure compliance with confidentiality, HIPAA, and federal residents' rights.

Requirements/Qualifications

• Graduated from an accredited Physical Therapy college or university.
• Valid professional license and registration for the state of practice.
• Experience with long-term care and/or geriatric care is preferred but not required.
• Experience with Case Mix Index and PPS scheduling is preferred but not required.
