Physical Therapist

At WelbeHealth each participant of our program is guided by our interdisciplinary team (IDT) – composed of clinical and non-clinical members who work cohesively to provide quality patient-centered care.

About the role

The Physical Therapist is a key member of the IDT, focusing on care planning and the coordination and delivery of restorative and rehabilitative physical therapy services to participants, working to maximize independence and safety.

• Lower patient volumes – no more seeing 3-4 patients an hour
• Form long-term relationships with participants rather than short-term rehab stints
• Collaborate with other therapists, providers, and disciplines

Responsibilities

• Conduct Physical Therapy assessments in our facility and occasionally in the home to define participant care needs, preferences, and goals
• Actively participate in IDT meetings to develop participant care plans
• Deliver and document interventions as agreed upon in participant care plans
• Provide active and passive exercise, muscle re-education, gait, functional training of ADLs, and treatments including massage, heat, sound, water, light, or electricity
• Supervise Therapy Assistants, providing training, instruction, and guidance needed to implement Functional Maintenance Programs (FMP)
• Provide training and instruction to staff, participants, and caregivers/families on the use and care of therapeutic equipment

Requirements

• Bachelor’s degree in physical therapy; master’s or doctoral degree preferred
• Valid California Physical Therapy License, or proof of license eligibility
• Must obtain CPR and First Aid certification within 180 days of hire and maintain certification thereafter; CPR and First Aid certification upon hire preferred
• Outpatient, acute, inpatient, or skilled nursing experience is ideal

Pay

Compensation is based on a per diem hourly rate of $58.63—$70.76 USD, determined by experience and other relevant qualifications.
