Physical Therapist

About the role

We are seeking licensed Physical Therapists to provide personalized, one-on-one care to older adults in their homes and senior living communities.

Responsibilities

• Conduct evaluations and develop individualized treatment plans
• Deliver evidence-based therapy to improve mobility, independence, and overall well-being
• Document patient progress accurately within electronic medical records
• Collaborate with interdisciplinary teams including PTs, OTs, SLPs, and fitness specialists
• Educate patients, caregivers, and staff on safety and aging in place strategies
• Provide telehealth and virtual care to enhance access and flexibility

Qualifications and Skills

• Valid Physical Therapy license or eligibility in the practice state
• Degree from an accredited Physical Therapy program
• Proficiency in basic computer use and EMR systems
• Current CPR certification

Physical Demands

• Performing moderate physical effort, including lifting up to 50 lbs occasionally
• Assisting with patient transfers and mobility
• Standing, walking, climbing stairs, and driving between patient locations
• Using PPE as required
