About the role
We are seeking licensed Physical Therapists to provide personalized, one-on-one care to older adults in their homes and senior living communities.
Responsibilities
- Conduct evaluations and develop individualized treatment plans
- Deliver evidence-based therapy to improve mobility, independence, and overall well-being
- Document patient progress accurately within electronic medical records
- Collaborate with interdisciplinary teams including PTs, OTs, SLPs, and fitness specialists
- Educate patients, caregivers, and staff on safety and aging in place strategies
- Provide telehealth and virtual care to enhance access and flexibility
Qualifications and Skills
- Valid Physical Therapy license or eligibility in the practice state
- Degree from an accredited Physical Therapy program
- Proficiency in basic computer use and EMR systems
- Current CPR certification
Physical Demands
- Performing moderate physical effort, including lifting up to 50 lbs occasionally
- Assisting with patient transfers and mobility
- Standing, walking, climbing stairs, and driving between patient locations
- Using PPE as required
