Physical Therapist

We are seeking a licensed Physical Therapist to deliver personalized, in-home and senior living care to older adults, primarily through our Geriatric House Calls™ model. Key Responsibilities Include Providing one-on-one physical therapy sessions in private residences and senior communitiesConducting evaluations, developing individualized treatment plans, and delivering evidence-based therapy to enhance mobility and independenceDocumenting care accurately within electronic medical recordsCollaborating with a multidisciplinary team of health professionalsEducating patients, caregivers, and staff to promote safety and aging in placeIncorporating telehealth and virtual care for greater flexibility and patient access Qualifications Include Valid Physical Therapy license and degree from an accredited programProficiency in basic computer and EMR systemsCurrent CPR certification Physical Requirements Ability to exert medium physical effort, assist with patient transfers, and perform therapeutic activitiesCapacity to drive, reposition patients, and wear PPE as needed This role offers flexible employment options, competitive compensation, benefits, and opportunities for professional growth within a supportive, clinician-driven environment.
