About the role

Provides direct supervision of physical therapy assistants in accordance with New York State Physical Therapy Practice Act. Participates in developing Individualized Family Service Plans, Individualized Education Programs, Individual Program Plans, or Individual Service Plans as appropriate for the individual. Evaluates, fits, adjusts, fixes and maintains prosthetic, orthotic and adaptive equipment devices and recommends client-specific modifications necessary for function at home and/or school. Participates in improving building accessibility and planning environmental modifications. Documents need, advocates and provides recommendations in ordering/purchasing equipment for agency use. Provides clinical educational opportunities for students of physical therapy. Completes incident reports as indicated. Conducts daily safety inspections to minimize and address safety hazards and ensure proper equipment usage.

Responsibilities

  • Instruct staff members and family in treatment procedures to be continued at home, school, or at Day Habilitation.
  • Consult with medical practitioners, service coordinators and appropriate others to plan, implement and assess interventions.
  • Participate with the service/treatment coordinators and other team members to develop Individualized Family Service Plans, Individualized Education Programs, Individual Program Plans or Individual Service Plans as appropriate for the individual.
  • Complete all documentation in compliance with applicable regulations and industry standards as needed for reimbursement.
  • Complete all documentation within timelines outlined by agency, departmental and regulatory standards.
  • Plan for the transition from Early Intervention to Preschool, to school-based services (BOCES), and then the community; preparing the individual to function as independently as possible away from therapy services as targeted outcomes are achieved.
  • Evaluate, fit, adjust, fix and maintain prosthetic, orthotic and adaptive equipment devices and recommend client-specific modifications necessary for function at home and/or school.
  • Provide clinical educational opportunities for students of physical therapy.
  • Complete incident reports as indicated.
  • Perform or ensure that a daily safety inspection of your site is conducted to minimize and address any possible safety hazards and to ensure all equipment is being used safely and properly.

Qualifications

Requires a Master’s degree in Physical Therapy from an accredited program. Must be licensed to practice as a Physical Therapist in the State of New York. Must have completed 36 contact hours of continuing education requirements as delineated by national PT credentialing agencies as well as any state requirements applicable over a 3-year renewal period to maintain credentials as an PT. Excellent time management, organizational, communication and training skills. Detail oriented. Proficient with the use of varied computer programs. Personal Effectiveness/Credibility Supervisory Responsibility - May be required to provide service supervision for a Physical Therapy Assistant that is licensed by the State of New York if assigned.

$50,000 - $60,000 annually.
