Seeking a licensed Physical Therapist to provide personalized, one-on-one care to older adults in their homes or senior living communities, utilizing a flexible schedule and evidence-based treatments.
Responsibilities
- Conduct evaluations and develop individualized care plans
- Deliver therapy to enhance mobility and independence
- Document care in EMR
- Collaborate with interdisciplinary teams
- Incorporate telehealth to expand access and flexibility
- Educate patients and caregivers
- Promote safety and support aging in place
Qualifications
- Valid physical therapy license or eligibility
- Degree from an accredited physical therapy program
- Computer proficiency
- Current CPR certification
Physical demands
- Lifting and repositioning patients
- Assisting with transfers
- Driving between locations
