Seeking a licensed Physical Therapist to provide personalized, one-on-one care to older adults in their homes or senior living communities, utilizing a flexible schedule and evidence-based treatments.

Responsibilities

• Conduct evaluations and develop individualized care plans
• Deliver therapy to enhance mobility and independence
• Document care in EMR
• Collaborate with interdisciplinary teams
• Incorporate telehealth to expand access and flexibility
• Educate patients and caregivers
• Promote safety and support aging in place

Qualifications

• Valid physical therapy license or eligibility
• Degree from an accredited physical therapy program
• Computer proficiency
• Current CPR certification

Physical demands

• Lifting and repositioning patients
• Assisting with transfers
• Driving between locations
