Role: Licensed Physical Therapist specializing in geriatric care, delivering personalized, one-on-one therapy to older adults in private homes and senior living communities. Key Responsibilities Conduct evaluations, develop individualized treatment plans, and provide evidence-based therapy to improve mobility, independence, and quality of life.Perform therapy sessions supporting patient wellness, functional movement, and aging in place.Document patient progress accurately in the EMR system, adhering to regulatory standards.Collaborate with interdisciplinary teams including PTs, OTs, SLPs, and Fitness Specialists.Educate patients, caregivers, and staff on safety and functional strategies.Utilize telehealth and virtual care to enhance access and flexibility. Qualifications & Skills Valid Physical Therapy license or eligibility in the practice state.Degree from an accredited PT program.Proficiency with computers and EMR systems.Current CPR certification. Physical Requirements: Ability to perform moderate physical effort, assist with patient transfers, reposition patients, and drive to various locations. Must wear PPE when required.
