About the role
Licensed Physical Therapist to provide one-on-one care to older adults in private residences and senior living communities using the Geriatric House Calls™ model.
Responsibilities
- Conduct evaluations and develop personalized care plans
- Deliver evidence-based therapy to support patient independence, mobility, and wellness
- Maintain accurate records in EMR systems
- Collaborate with an interdisciplinary team including PTs, OTs, SLPs, and fitness specialists
- Educate patients, caregivers, and staff
- Incorporate virtual care to enhance access and continuity
Qualifications
- Valid Physical Therapist license
- Degree from an accredited Physical Therapy program
- Basic computer skills
- Current CPR certification
Physical demands
- Ability to perform moderate physical effort
- Assist with patient transfers
- Drive to patient locations while wearing PPE as needed
