About the role
Licensed Physical Therapist providing personalized in-home and senior living care, with optional telehealth services.
Responsibilities
- Conduct evaluations and develop individualized care plans.
- Deliver evidence-based therapy to improve mobility, independence, and quality of life for older adults.
- Document patient progress accurately and collaborate with interdisciplinary teams.
- Educate patients, caregivers, and staff on safety and aging-in-place strategies.
- Support virtual care when appropriate.
Qualifications & Skills
- Valid PT license and degree from an accredited program.
- Proficiency with EMR and computers.
- CPR certification.
- Ability to meet physical demands including lifting, repositioning patients, and driving to care locations.
Key Duties
- Provide one-on-one therapy sessions in various settings.
- Perform assessments and document care.
