Physical Therapist

About the role

Provide one-on-one physical therapy to older adults in private residences and senior living communities, focusing on improving mobility, independence, and quality of life through evidence-based treatment and personalized care plans.

Responsibilities

• Conduct evaluations and develop individualized treatment plans
• Deliver therapy sessions and document within EMR systems
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists
• Educate patients, caregivers, and staff to promote safety and aging in place
• Offer virtual care options, including telehealth, to enhance continuity and access
• Build strong patient relationships and contribute to community wellness

Qualifications

• Valid Physical Therapist license
• Degree from an accredited Physical Therapy program
• Computer proficiency
• Current CPR certification

Physical demands

• Exert medium effort in therapy delivery
• Assist with patient transfers as needed
• Drive to care locations
