About the role
Provide one-on-one physical therapy to older adults in private residences and senior living communities, focusing on improving mobility, independence, and quality of life through evidence-based treatment and personalized care plans.
Responsibilities
- Conduct evaluations and develop individualized treatment plans
- Deliver therapy sessions and document within EMR systems
- Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists
- Educate patients, caregivers, and staff to promote safety and aging in place
- Offer virtual care options, including telehealth, to enhance continuity and access
- Build strong patient relationships and contribute to community wellness
Qualifications
- Valid Physical Therapist license
- Degree from an accredited Physical Therapy program
- Computer proficiency
- Current CPR certification
Physical demands
- Exert medium effort in therapy delivery
- Assist with patient transfers as needed
- Drive to care locations
