About the role
We are seeking a Physical Therapist to provide skilled physical therapy services to patients across Senior Living Communities. In this role, you will be instrumental in helping seniors recover, enhance mobility, and maintain their independence through tailored physical therapy treatment plans. Responsibilities
- Evaluate physician’s referral and patient’s medical record.
- Assess and evaluate therapeutic/rehabilitative/functional status using approved evaluation techniques, tests, and observations, and participate in the development of the total plan of care.
- Evaluate the home environment.
- Determine priority needs for physical therapy.
- Direct physical therapy treatment.
- Report to physician patient’s response to treatment and changes in condition.
- Identify patient and family/caregiver needs for other home health services and refer as necessary.
- Train patients in the use of prosthetic devices as needed.
- Administer a prescribed physical therapy program to restore and maintain maximum function and prevent disability using a range of therapeutic modalities.
- Maintain appropriate documentation in the patient chart and re-evaluate patient progress, revising treatment programs when necessary.
- Communicate physical therapy philosophy, treatment approaches, and goals to patients, families, and other members of the rehabilitation team.
- Supervise Physical Therapy Assistants according to Agency policy and state regulations.
Requirements
- Valid Physical Therapist license in Arizona, in good standing.
- Two years of Home Health experience as a Physical Therapist preferred.
- Possesses and maintains current CPR certification.
- Licensed driver with reliable vehicle and current vehicle insurance.
- Experience in geriatric physical therapy or working with seniors is preferred.
- Ability to work independently and manage time efficiently.
- Excellent observation, verbal and written communication skills, problem-solving skills, and organizational skills.
- Excellent communication and interpersonal skills to work effectively with residents, families, and the healthcare team.
Schedule
PRN to cover the West Valley & East Valley.
