Physical Therapist

About the role

This position involves providing personalized, one-on-one physical therapy to older adults in private residences and senior living communities, utilizing evidence-based treatments to enhance mobility, independence, and quality of life.

Responsibilities

• Conduct evaluations and develop individualized care plans
• Deliver therapy sessions supporting patient wellness and functional independence
• Document care activities accurately in EMR systems
• Collaborate with interdisciplinary teams including PTs, OTs, SLPs, and fitness specialists
• Educate patients, caregivers, and staff on safety and aging-in-place strategies
• Incorporate telehealth or virtual care to extend reach and flexibility

Qualifications

• Valid Physical Therapy license and degree from an accredited program
• Proficiency in computer systems and EMR
• Current CPR certification
• Ability to meet physical demands such as lifting, repositioning, and assisting patients
• Valid driver’s license for travel to patient locations
