Physical Therapist

Role Overview: A licensed Physical Therapist providing personalized, in-home and senior living care to older adults. The role involves evaluating patients, developing individualized treatment plans, and delivering evidence-based therapy to enhance mobility, independence, and overall quality of life. Responsibilities Include Offering one-on-one therapy sessions in private residences and senior living communitiesPerforming evaluations, creating care plans, and delivering evidence-based treatmentsSupporting patient wellness, longevity, and functional independenceDocumenting care accurately within EMR systemsCollaborating with interdisciplinary teams including PTs, OTs, SLPs, and Fitness SpecialistsEducating patients, caregivers, and staff on safety and aging-in-place strategiesUtilizing telehealth/virtual care to extend access and flexibility Qualifications & Skills Valid Physical Therapy license and degree from an accredited programProficiency with basic computer and EMR toolsCurrent CPR certificationAbility to meet physical demands such as lifting, repositioning, and assisting patients Key Attributes: Autonomy, flexibility, strong communication skills, and a passion for improving patient outcomes in community settings.
