About the role

Seeking a Physical Therapist Assistant responsible for implementing treatment plans, providing exercise instruction, and patient education to enhance patient well-being in a home care setting. The role involves following state-specific laws, performing qualified procedures, and collaborating with the healthcare team.

Responsibilities

• Follow treatment plans set by supervising Physical Therapists and recommend adjustments when necessary.
• Instruct and assist patients with active/passive exercises, gait, ADL, transfer, safety, and prosthetic training.
• Monitor, document, and report patient responses and condition changes to the supervising PT and other team members.
• Educate patients, families, caregivers, and healthcare team members within the scope of practice.
• Adapt to diverse home environments and patient needs.

Qualifications & Skills

• Current licensure, registration, or certification to practice physical therapy in the state.
• Valid driver’s license, vehicle insurance, or access to transportation.
• Availability to work weekend rotations.
• Preferred: CPR certification and physical ability to perform patient care activities.

Pay
