About the Role

A Physical Therapy Assistant provides skilled education, treatment, and interventions under a physician's plan of care in home health settings. Collaborating with interdisciplinary teams, they communicate updates to optimize patient well-being.

Responsibilities

• Deliver direct physical therapy services per therapist and physician instructions.
• Educate patients and caregivers on treatment plans, safety, and device use.
• Monitor and report patient condition changes to supervisors and physicians.
• Assist in pain relief, functional development, and rehabilitation.
• Coordinate discharge planning and provide follow-up instructions.
• Participate in in-service programs and adhere to professional standards.
• Maintain confidentiality, accurate documentation, and timely reporting.

Requirements

• Graduation from an accredited Physical Therapist Assistant program.
• Current, unrestricted state license and CPR certification.
• Strong communication, collaboration, and professional conduct.
