Physical Therapist Assistant - Full Time

About the role

Seeking a Physical Therapy Assistant to provide skilled education, treatments, and interventions under the direction of a physical therapist and physician, primarily in home health settings.

Responsibilities

• Deliver direct physical therapy services, treatments, and procedures as per the care plan.
• Communicate regularly with patients and caregivers about the plan of care.
• Report patient observations and changes to supervisors and physicians.
• Assist in relieving pain, restoring function, and enhancing performance.
• Educate patients and caregivers on safe device use and rehabilitation techniques.
• Collaborate with interdisciplinary team members to discuss and plan patient care.
• Prepare discharge plans and instructions, ensuring smooth transitions.
• Maintain accurate documentation, including progress notes and daily reports.
• Participate in in-service programs and adhere to professional standards and confidentiality protocols.
• Provide skilled therapy aligned with the care plan.
• Educate and communicate effectively with patients and team members.
• Monitor and report patient progress and conditions.
• Ensure compliance with ethical, professional, and confidentiality standards.

Qualifications & Skills

• Graduation from an accredited Physical Therapist Assistant program.
• Current, unrestricted state license as a Physical Therapy Assistant.
• Valid CPR certification.
• Strong communication, collaboration, and documentation skills.
