Position Overview

Seeking a dedicated Physical Therapy Assistant to provide skilled education, treatment, and interventions under a physician’s plan of care in a home health setting. Collaborate with an interdisciplinary team, communicate updates, and ensure patient safety and well-being.

Key Responsibilities

• Deliver direct physical therapy services as directed and in accordance with the physician’s plan.
• Educate patients and caregivers on treatment, device use, and rehabilitation techniques.
• Monitor and report patient progress, conditions, and reactions to treatments.
• Develop discharge plans and communicate instructions to patients and families.
• Coordinate with other disciplines to optimize care.
• Ensure documentation accuracy, confidentiality, and compliance with professional standards.
• Participate in ongoing education and perform other duties as assigned.

Qualifications And Skills

• Graduate from an accredited Physical Therapist Assistant program.
• Current, unrestricted state license.
• Proficiency in assessments, documentation, and policy adherence.
• Valid CPR certification and driver’s license.
