About the role
Provide comprehensive pharmaceutical care for the PACE (Program of All-Inclusive Care for the Elderly) population, supporting adults 55+ in maintaining independence and aging safely in their homes.
Responsibilities
- Review medication orders to identify, prevent, and resolve drug-related problems while recommending appropriate formulary alternatives to optimize patient outcomes.
- Accurately compound and dispense medications, ensuring safe and timely delivery for a medically complex geriatric population.
- Educate and counsel patients, caregivers, and interdisciplinary healthcare teams on medication therapies, adherence, and disease management.
- Collaborate with physicians and care teams while supervising pharmacy support staff and assisting with training/precepting pharmacy students as needed.
Requirements
- Licensure / Certification / Registration: Pharmacy Tech credentialed from the Pharmacy Technician Certification Board (PTCB) preferred. Must possess all state and local qualifications.
- Education: High School diploma equivalency OR 1 year of applicable cumulative job specific experience required.
- Note: Required professional licensure/certification can be used in lieu of education or experience, if applicable.
Preferences
- Previous pharmacist experience in a PACE (Program of All-Inclusive Care for the Elderly) setting preferred.
- Experience working with adults aged 55+ and geriatric populations strongly preferred.
- Prior long-term care (LTC) pharmacy experience preferred.
