John Muir Health’s Pharmacotherapy Clinic seeks an Advanced Practice Pharmacist (APP) to deliver comprehensive medication management for patients with complex, chronic conditions. Under a California Board of Pharmacy–approved Collaborative Practice Agreement (CPA), the APP prescribes, initiates, adjusts, and discontinues medications to optimize therapeutic outcomes across multiple specialties.
Responsibilities
- Assess medication regimens for safety, efficacy, and adherence.
- Streamline therapies, mitigate drug interactions, and educate patients and caregivers.
- Perform clinical assessments, order and interpret laboratory tests, and monitor patient responses.
- Collaborate with physicians and the interdisciplinary care team to ensure continuity of care and achievement of clinical goals.
- Document patient encounters and communicate findings in both patient-facing and team settings.
- Contribute to policy development, quality improvement initiatives, and staff education.
- Supervise pharmacy technicians, support regulatory compliance, and assume leadership duties in the absence of pharmacy management.
Requirements
- Doctor of Pharmacy (PharmD) or Bachelor of Science in Pharmacy (BSPharm) from an ACPE-accredited school.
- Current California Registered Pharmacist (RPH) license.
- Basic Life Support (BLS) certification from the American Heart Association.
- Advanced Practice Pharmacist (APh) license required within six months of hire.
- Board certification in a relevant specialty (e.g., BCACP, BCPS, BCGP, BCOP) required for the assigned practice area.
- Must meet California State Board of Pharmacy requirements for APP recognition by fulfilling any two of the following:
- Current board certification in a clinical area (e.g., BCACP, BCPS, BCGP, BCOP).
- Completion of an ASHP-accredited PGY-1 or PGY-2 residency in Ambulatory Care, Pharmacotherapy, Cardiology, Internal Medicine, Oncology, Geriatrics, or Specialty Pharmacy.
- At least 1,500 hours of direct patient care under a CPA or similar protocol within the past 10 years.
- For candidates without residency training, a minimum of five years of clinical experience in ambulatory care or chronic disease management is strongly preferred.
- Ability to multitask in a high-acuity outpatient environment.
- Experience educating patients, monitoring labs, and managing metabolic disease states.
- Experience developing policies and leading continuous process improvement.
Pay
Hourly range: $99.60 – $149.39. Offer amounts are based on demonstrated experience and licensure. Pay will be adjusted to the local market if hired outside the Bay Area.
Schedule
- Shift: 08:00 – 17:00 (exempt position).
- Scheduled weekly hours: 40.
