Pharmacotherapy Pharmacist - Rheumatology/GI - Pharmacotherapy Clinic - Full Time - 8 Hour - Days

Responsibilities

  • Assess medication regimens for safety, efficacy, and adherence
  • Streamline therapies when appropriate
  • Mitigate drug interactions
  • Education patients and caregivers to support long-term treatment success
  • Perform clinical assessments, order and interpret laboratory tests, and monitor patient responses to therapy
  • Collaborate closely with physicians and the care team to ensure continuity of care and achievement of clinical goals
  • Document and communicate effectively in both patient-facing and interdisciplinary team settings
  • Contribute to policy development, quality improvement, and staff education
  • Supervise technicians, support regulatory compliance initiatives, and assume leadership duties in the absence of pharmacy management

Requirements

  • Doctor of Pharmacy (PharmD) or Bachelor of Science (BSPharm) from a school accredited by ACPE - Required
  • Candidates must meet qualifications consistent with California State Board of Pharmacy requirements for Advanced Practice Pharmacist (APP) recognition by fulfilling any two (2) of the following:
    • Current board certification in a relevant clinical area (e.g., BCACP, BCPS, BCGP, BCOP, or other board-approved specialty certification)
    • Completion of an American Society of Health System Pharmacists (ASHP) accredited PGY-1 or PGY-2 residency in Ambulatory Care, Pharmacotherapy, Cardiology, Internal Medicine, Oncology, Geriatrics, Specialty Pharmacy, or other applicable specialty
    • At least 1,500 hours of direct patient care experiences within the past 10 years under a CPA or similar structured clinical protocol
    • For candidates without residency training, a minimum of five (5) years of clinical experience in ambulatory care or chronic disease state management is strongly preferred
  • Ability to multitask and adapt to a high-acuity outpatient environment
  • Demonstrated ability to perform in a complex and collaborative clinical setting
  • Must be able to educate, monitor, order, and follow labs for patients, related to metabolic disease states
  • Experience in developing policies and engaging in continuous process improvement

Qualifications

  • RPH Registered Pharmacist - California Board of Pharmacy - Required
  • BLS Basic Life Support - American Heart Association – Required
  • APh Advanced Practice Pharmacist License – Required within six months of hire
  • Board Certification in a pharmacy specialty recognized by the Board of Pharmacy Specialties – Required for positions where it is relevant to the assigned area of practice (e.g., BCOP for oncology, BCPS for pharmacotherapy, BCACP for ambulatory care, etc.)
