About the role

The Clinical Pharmacist – SRT contributes to AHF by being assigned to more than one location, based on scheduling and geographical needs. The role expands the primary functions of a pharmacist to provide individualized health promotion and disease prevention, including vaccine administration, long‑acting HIV medicine administration, Diabetes monitoring, Nicotine reduction programs and other subject‑matter areas, in collaboration with other AHF providers. The position enhances AHF’s reputation by improving patient outcomes, enhancing current services, expanding opportunities for professional growth and satisfaction, and assisting with the ongoing development of the AHF Healthcare System. As a Clinical Pharmacist – SRT you will be a passionate advocate for AHF’s top initiatives and must keep the AHF Core Values and Mission top of mind in all you do. AHF is nimble and able to adapt in a dynamic environment to provide the best experience for clients and a supportive workplace for employees, and operates with a collaborative organizational structure where staff are accountable to multiple leaders.

Responsibilities

• Provide individualized health promotion and disease prevention, including vaccine administration, long‑acting HIV medicine administration, Diabetes monitoring, Nicotine reduction programs and other subject‑matter areas.
• Collaborate with other AHF providers to deliver these services.
• Improve the outcomes of AHF patients and enhance current AHF services.
• Expand opportunities for professional growth and satisfaction.
• Assist with the ongoing development of the AHF Healthcare System.
• Act as a passionate advocate for AHF’s top initiatives.
• Ensure that AHF Core Values and Mission remain top of mind in all activities.
• Adapt to a dynamic environment and work within AHF’s nimble, collaborative structure where staff are accountable to multiple leaders.
• Work as part of a close‑knit team to provide individualized health promotion and disease prevention, including administration of immunization, Diabetes and Smoking Cessation programs, and other subject‑matter expertise (SME) programs developed by AHF.
• Exercise appropriate collaborative drug‑therapy management authority.
• Exercise prescriptive authority where legally and organizationally authorized, such as for PrEP and PEP therapies.
